This web site is for a doctor's office in Hamden, CT. The web site is a contact point for potential patients to get information about Dr. Farmer and Internal Medicine & Family Practice.
This web site was created using a medical image map from a template, modified to suit this project. All of the images were created or obtained by CT Internet Technology. The site is comprised of segmented image maps, translated into html. A section containing PDF documents was also included in the web site to allow patients to print out forms prior to their visit.
